In the box labeled Street Address, enter your number and street name, as well as any other relevant information, such as suite or apartment number. You do not need to include your institution's name in this box.
In the box labeled City, enter the name of your city.
U.S. residents select your state from the drop-down State menu by clicking on the down arrow and choosing your state. You must also select USA in the Country drop-down menu.
Non-U.S. residents select your country from the drop-down Country menu. If your country is not listed, enter it in the Other country name box.
In the box labeled Postal code, enter your postal or zip code.
