Nation's healthcare leaders to announce new NCQA criteria for PCMH Model at healthcare summit

Washington—The Patient Centered Primary Care Collaborative (PCPCC), a coalition representing business leaders, policymakers and the nation’s 330,000 primary care physicians, will announce their intent to promote new voluntary criteria for recognition of physician practices to be designated as a patient-centered medical home on Wednesday, November 7 at a national health care summit in Washington, DC. The patient-centered medical home model is aimed at re-centering health care on the patient’s needs and priorities by providing personalized primary and preventive care.

The one-day summit will feature expert speakers, including business leaders, policymakers and health officials advocating for the nation to adopt the patient-centered medical home model.
