In the gut mucosa of IgAN patients, expanded IgA+ plasma cells highly express BCMA, supported by APRIL signals, leading to excess pathogenic IgA and kidney injury. Therapeutic depletion of these BCMA+ cells cuts off this survival signal, reducing IgA production and alleviating kidney damage.
