Rethinking oral antibiotics after pediatric appendicitis. This graphical summary shows the study design and key findings of a systematic review and meta-analysis on post-discharge oral home antibiotics (OHA) in children with complicated acute appendicitis (CAA). Across 14 observational studies, OHA did not show consistent benefits in reducing intra-abdominal abscesses, organ/space infections, or readmissions, while exposure-restricted analysis suggested a possible increase in readmission risk. The evidence was rated as very low certainty, highlighting the need for high-quality multicenter randomized controlled trials (RCTs) to clarify whether routine OHA after discharge is necessary.
