Ct-HBx promotes HBV-HCC development by enhancing chromatin accessibility to upregulate PAI-1, which is a novel biomarker for HBV-HCC. (IMAGE)

Ct-HBx is more potent than WT-HBx in upregulating histone acetyltransferases (GCN5, KAT6A) and chromatin remodeling factors (BAF155, ATRX), enhancing chromatin accessibility of the -1kb promoter and the 3’UTR of PAI-1 to promote PAI-1 expression. PAI-1 functional SNPs (rs2227631-G, rs11178-T, rs1050955-A) increase its expression and interact with HCC-risk HBV mutations to synergistically elevate HBV-HCC susceptibility. Serum PAI-1 levels gradually rise during HBV-induced hepatocarcinogenesis. Higher expression of PAI-1 predict poor prognosis in HBV-HCC, identifying PAI-1 as a novel prognostic and predictive biomarker for HBV-HCC.
