Exosomes are formed within multivesicular bodies and released into the extracellular space carrying selectively sorted miRNAs. These vesicles deliver regulatory signals to recipient cells, facilitating immune homeostasis, inter-organ communication, and responses to cancer and infectious diseases. Understanding how miRNAs are packaged, transported, and functionally delivered remains a key challenge in advancing EV-based diagnostics and therapeutics.
